Required Documents for UPI Credit Card Renewal

The following documents are required to renew the UPI Credit Card for each employee type. 

  1. Employees 
    ■ Pay Slip for the last month
    ■ NRC (Front and Back) 
    ■ Census Data
    ■ Ward recommendation (or) Electricity Bill (within one month) 
    ■ Recommendation from HR or Employer (Original) (within one month) 
    ■ Bank statement for 6 months
    ■ Auto Debit Instruction
    ■Two Guarantors
  1. Self-Employed
    ■ Income with Bank Statement 
    ■ NRC (Front and Back) 
    ■Census Data
    ■ Ward recommendation (or) Electricity Bill (within one month) 
    ■ Income must be submitted by KBZ bank account only
    ■ Company license / Company Extract 
    ■ Auto Debit Instruction
    ■ Two Guarantors 
  1. KBZ Bank Staff 
    ■ NRC (Front and Back) 
    ■ Census Data
    ■ Ward recommendation (or) Electricity Bill (within one month) 
    ■ Auto Debit Instruction
    ■ VC/FC head approval letter (within one month) 
    ■ HR recommendation letter (within one month) 
    ■ Consent Form
    ■ No Guarantors

Information about Guarantor 

If the Guarantor is an employee, the guarantor’s salary in the past 6 months should be higher (or) the same as the applicant’s salary. 

Required Documents for Guarantor 
■ NRC (Front and Back) 
■ Census Data
■ Ward recommendation (or) Electricity Bill (within one month) 
■ Recommendation from HR (Original) (within one month)   
■ Pay Slip for the last month

If the Guarantor is self-employed, the guarantor’s past 6 months income should not be less than the applied credit amount. 

Required Documents for Guarantor 
■ Bank Statement for the last 6 months (Income must be submitted by KBZ bank account only)
■ NRC (Front and Back) 
■ Census Data
■ Ward recommendation (or) Electricity Bill (within one month) 
■ Company license / Company Extract
